NOTE: Fosamprenavir approved in the US - October 2003. Fosamprenavir is a pro-drug of amprenavir. This drug requires fewer pills than amprenavir (4 pills a day) without food restrictions. European approval is expected mid 2004. Fosamprenavir remains on expanded access. Information for doctors and patients: www.lexiva.com AMPRENAVIR (APV) Agenerase® Approved for use in HIV infection in combination with other antiviral drugs DESCRIPTION: Amprenavir is a protease inhibitor. DOSE: 1200 mg (8 x 150 mg capsules) twice daily with or without food (Not a high fat meal). Liquid, 20 mg/kg twice daily or 17 mg/kg three times daily, with or without food. Contains Vit. E (109 IU) per 150 mg capsule. Adolescents / Paediatric: Oral solution: 20 mg/kg twice daily or 17 mg/kg three times daily (max 2.8 g a day). Liquid formulation for children over 4 years. Neonates: No data for children under 4 years old. SIDE EFFECTS: Nausea, diarrhoea, rash, tingling around the mouth, headache, fatigue and vomiting. 4% of people had increased ALT/AST levels indicating liver toxicity. Less than 1% had Stevens-Johnson syndrome. Amprenavir may be associated with acute haemolytic anaemia, diabetes mellitus, and hyperglycaemia. RESISTANCE: There is cross resistance between APV and other PIs, however people resistant to current PIs may benefit from the use of APV in rescue treatment. INTERACTIONS: Amprenavir with astemizole, bepridil, cisapride, dihydroergotamine, ergotamine, terfenadine, midazolam, flurezapam, diazepam and triazolam risks serious or life-threatening adverse events. Contraindicated with St John's Wort. Rifampin reduces concentrations of APV by about 90%. Rifabutin reduces APV by about 14%, but rifabutin is increased 3- to 6-fold by APV. APV with indinavir increases APV by 64% and by up to 31%. Efavirenz decreased APV by 36%. |
